URL Changes to Legacy Functions
For network security purposes, we are changing the web addresses for some of the legacy forms and tools that users currently access on the slforms.universalservice.org domain. We will automatically redirect users to the new address for each form or tool, but we suggest that you update any bookmarks that you have created for these forms and tools.
Earlier this week, we changed the web addresses for the following four legacy tools:
-
FCC Form 470 Download Tool – You can use this tool to search for FCC Forms 470 filed before EPC was available, including some FY2016 forms and all forms for earlier funding years.
-
FCC Form 471 Status Tool – You can use this tool to find the status of FCC Forms 471 completed or certified for FY2015 and earlier by a specific billed entity number (BEN) in a single funding year.
-
Search for BEN Information Tool – You can use this tool to search for information on BENs by entity type, full or partial name, zip code, or state/territory. This tool contains some legacy information that may not be available in the Entity Download Tool.
-
SPIN Search Tool – You can use this tool to search for information on Service Provider Identification Numbers (SPINs) by name or SPIN. This tool contains some legacy information that may not be available in the Service Provider Download Tool.
We will continue to provide information on updated web addresses for legacy forms and tools as these changes are implemented.
